H.E.R.S. Umpire Clinic Permission, Waiver, and Code of Conduct Agreement

I, the parent or legal guardian of __________________ (“Child”), or I, the undersigned participant (if 18 years of age or older), hereby give permission for the child named above or myself to participate in any and all H.E.R.S. activities during the Umpire Clinic.

I understand that I am responsible for providing transportation to and from clinic activities. I agree to notify a H.E.R.S. commissioner or clinic representative as far in advance as possible if the participant will be absent from the clinic.

I acknowledge and assume all risks and hazards incidental to participation in the Umpire Clinic, including transportation to and from clinic activities. I hereby waive, release, absolve, indemnify, and agree to hold harmless the H.E.R.S. organization, its sponsors, supervisors, commissioners, participants, and individuals providing transportation to or from clinic activities from any claims covered by accident or liability insurance.

H.E.R.S. maintains a zero-tolerance policy for any form of bullying, harassment, or unsportsmanlike conduct, whether in person or through electronic communication, during or related to clinic activities. Any participant who does not adhere to this policy may be dismissed from the clinic and prohibited from future participation in H.E.R.S. activities.

I understand that H.E.R.S. provides Excess Medical Liability Insurance coverage for each participant.

Photo, Video, and Audio Release Consent

I, the parent or legal guardian of __________________ (“Child”), or I, the undersigned participant (if 18 years of age or older), hereby grant H.E.R.S. permission to use the child’s or my photograph, video recordings, audio recordings, and/or written or spoken words in connection with the H.E.R.S. Umpire Clinic.

I grant full, irrevocable, and perpetual consent to H.E.R.S., its affiliates, commissioners, representatives, and assignees (collectively, the “Grantees”) to reproduce, distribute, publish, or display such materials worldwide in any format or media now known or later developed.

The Grantees may use the participant’s name, likeness, image, voice, or statements, in whole or in part, in connection with promotional materials, advertising, educational materials, social media, research, presentations, or other media produced by H.E.R.S. Materials may be used with or without the participant’s name. The Grantees may make reasonable edits to testimonial or recorded content, provided the overall spirit and intent of the remarks are preserved.

I waive any right to inspect or approve the finished materials and release the Grantees from any claims or liabilities, including but not limited to claims for invasion of privacy, defamation, or misappropriation, arising from the use of these materials.
